Dear Colleagues,

The North York Toronto Health Partners (NYTHP) is North York’s Ontario Health Team (OHT), one of Ontario’s first OHTs announced in December 2019; a collaborative of over 21 Core Partners and 35 Alliance Partners working together to streamline access to healthcare and meet the needs of North Yorkers. As OHTs continue to develop in the province, the importance of a unified primary care voice at OHT tables has grown. Several OHTs have developed formal partnerships across primary care providers, and given North York’s precedence of a strong primary care community is following in this direction.

As NYTHP Primary Care develop the foundation of NYTHP’s Primary Care Association, NYTHP’s Primary Care Leads will be hosting 2 informational webinars on Wednesday February 3^rd and 17^th, 2021 from 5-6pm (registration links below) in order to gain your feedback on the attached documents: NYTHP’s Primary Care Association Agreement and the Terms of Reference of our Primary Care Association.

Following these webinars, Primary Care Providers will have an opportunity to sign on to the association between February 3^rd and February 28^th; during this time, providers interested in partaking at the association table are asked to indicate their interest, at which point new association members will vote on association leadership between March 1^st and March 15^th 2021. We encourage you to consider running for an elected position on the Primary Care Advisory Council. If you are interested, please review the attached Terms of Reference (TOR). Information will be sent out next week with regards to expressing your interest for a Primary Care Advisory Council position.

North York Toronto Health Partners’ Primary Care Association will be announced on March 1^st 2021, and the Primary Care Advisory Council will start its function on April 1^st, 2021.
